• Forms requiring a detailed narrative
   
• Forms needing review of clinical history and supporting documentation

• Payment is due at the time forms are submitted
   
• Standard processing time is 5–10 business days
   
• Form fees are not covered by insurance
   
• Forms are completed only for established patients who have been seen within the past 12 months
   
• Form completion is at the provider’s clinical discretion. Additional appointments may be required for forms requiring detailed narratives or disability documentation
   
• Forms will not be completed for new patients who have not yet been seen, or for individuals who are not active patients of the clinic
   
• Forms must be complete and legible when submitted. Incomplete paperwork may delay processing
   
• Patients may be asked to schedule an appointment if more clinical information is needed to complete the form accurately

• Form completion requires provider time for chart review, clinical accuracy, documentation, and compliance.
   
• Fees reflect the standard rates of NP, PA, and MD practices in Arizona.
   
• Expedited and urgent fees compensate for prioritizing paperwork ahead of standard workflow.
